Release of Liability: I, (the name above), in due and lawful consideration for my child/children being permitted to take part in activities of the Cypress Homeschool Association, do hereby fully and completely release and waive any and all rights and claims that I, my heirs, devisees, guardians, legal representatives, subrogees, and assigns might now or hereafter have against Cypress Homeschool Association for personal injury, including death, property damage, or any other right or claim whatsoever, however, caused, and that we do hereby agree for ourselves, corporately or individually, to hold the Cypress Homeschool Association harmless with respect to all injuries or damages arising out of or as a result of entering into the activities of Cypress Homeschool Association whether caused, in whole or in parts by negligence or other acts of Cypress Homeschool Association. In addition, I also grant to the Cypress Homeschool Association’s authorized representatives, the right to use, reproduce, adapt, distribute, publish and publicly display the negatives and photograph(s) of my child(ren) and myself taken at CHA events for CHA publications and a members-only website. I have carefully read the agreement and fully understand its content. I am aware that this is a release and waiver of liability and claim and a contract between myself and Cypress Homeschool Association, and I have signed it of my own free will.
